In 2017, Dr. Kim Nero and her son, Oscar, moved to Port Charles. Kim took a job at Mercy Hospital while Oscar settled in at a new high school where he befriended Josslyn Jacks. The teenagers soon struck up a romantic relationship and started dating, so Carly Corinthos was eager to meet Oscar's mother. Oscar kept putting it off because his mother worked long hours, but Carly forced the issue by inviting Oscar and his mother to Thanksgiving with the Corinthos family. To Carly's delight, Kim accepted the invitation.
Carly worried that Kim might not realize that Oscar was dating the stepdaughter of a crime lord, so she was determined to put her best foot forward and win Kim over before Kim found out. On Thanksgiving, Kim arrived for dinner and quickly made it clear that she was aware of who Josslyn's stepfather was. Kim admitted that she didn't approve of Sonny's lifestyle. However, she also recognized that Oscar and Josslyn were young, and the romance likely wouldn't last, so Kim was willing to let things run their course. Moments later, Jason Morgan arrived for dinner.
Kim was stunned when she saw Jason because she recognized him as Andrew Cain. Carly immediately perked up because it had recently been revealed that Jason and Drew were identical twin brothers who had been separated at birth. Carly explained that there was a dispute about which twin was the real Jason Morgan because Drew had been critically injured in an auto accident, resulting in facial reconstruction surgery and amnesia. Carly was eager to know what Kim knew about Drew, but Kim claimed that she'd only known Drew for a short time.
According to Kim, she had met the Navy SEAL in San Diego, and they'd casually dated for a few months until he'd been deployed. They had always known that the relationship wouldn't last, so Kim had lost touch with Drew after he'd left San Diego. She admitted that she'd tried to reach out to Drew through the Navy to tell him about Oscar, but she'd been told that he had deserted, and eventually, she'd given up. Years later, Kim had decided to run Oscar's DNA through an ancestry site in the hopes of finding Drew or someone from Oscar's father's side of the family, because Oscar had expressed a curiosity about his father.
Kim claimed that she'd received feedback from the site, indicating that Oscar had family in Port Charles, so Kim had packed up her son and moved to New York in the hopes of finding Drew or his family. Kim was stunned when she realized that Oscar was a Quartermaine because Kim had moved her practice from Mercy Hospital to General Hospital, which meant that her new boss was Oscar's grandmother. Monica was delighted that she had another grandson and welcomed both Kim and Oscar to the family with open arms.
Kim decided to visit Drew and tell him about Oscar. Drew had no memory of Kim or the relationship they'd had, but he was eager to build a relationship with his son. Kim gave Drew a copy of a music CD that he'd made for her when he'd left San Diego in the hopes that it might help spark a memory of his past. Drew was grateful, but it was clear that his heart belonged to Sam. Kim focused on her life in Port Charles and struck up a friendship with Julian Jerome.
On May 28, 2019, Drew and Kim arranged to take their son's ashes to the top of Mt. Kilimanjaro. They thought it would be fitting for that to be his final resting place, since he had always dreamed of going. Monica supported the decision, but she asked Drew if he would mind if she put a plaque bearing her grandson's name in the family crypt, anyway. Drew was touched by the offer.
Julian was attracted to Kim, but he also understood that she was still in love with Drew. Kim was equally drawn to Julian, who was nursing a broken heart after his relationship with his ex-wife, Alexis Davis, fell apart. Julian and Kim enjoyed each other's company and decided to date casually. Kim called Julian "Charlie" because he owned Charlie's Pub, and she had initially assumed that was his name when she had first met him.
As the relationship with Julian flourished, Kim was able to move past Drew even when his relationship with Sam fell apart. She was pleased that Oscar and Drew had developed a close relationship, but everyone's life was upended when Oscar began to suffer headaches and fainting spells in August 2018.
Kim was forced to confide to Drew that their son had suffered several seizures in 2016 that had led to a diagnosis of inoperable Grade IV Glioblastoma. Kim had opted not to tell Oscar -- or anyone else-- about the malignant brain tumor because she had wanted Oscar to live life to the fullest during the short time that he had without the burden of knowing that he was terminally ill. Oscar was furious when he learned about his mother's deception, and her plans to force Oscar to undergo experimental treatment.
Oscar took steps to become an emancipated minor because he wanted to make his own choices about his treatment. He moved into the Dawn of Day house and took a job as a busboy at Charlie's Pub. Julian wanted Oscar to have a safe place to work that would accommodate his illness as Oscar's health steadily declined. Eventually, Josslyn got wind of Oscar's condition, and she realized that he had ended their relationship to spare her the pain of losing him. Josslyn talked Oscar into reconciling with his parents and undergoing the experimental treatment, but it only bought him a few precious weeks.
In Oscar's final days, Kim and Oscar moved into the Quartermaine mansion because Oscar wanted to be surrounded by family. On May 1, 2019, Oscar passed peacefully away in bed as Josslyn slept nearby in a chair. Kim was devastated, and she leaned Drew through the memorial service and a touching dedication to Oscar in the park.

Kim's life began to crumble as soon as she returned home. She missed her son, and she began to have disturbing visions of Oscar. Kim decided that a baby would end her unbearable pain, so she arranged a special dinner for Drew then invited him over. Shocked by Kim's desire to have another baby with him, Drew gently reminded her that a baby couldn't replace their son. Desperate, Kim turned to Julian, but he was equally reluctant to start a family with Kim while she was still reeling from Oscar's death.
Undaunted, Kim procured a sedative then tucked it away when she attended Liz and Franco's wedding reception on the Haunted Star. Kim took steps for the lights to go out then lured Drew to the fuse box where she injected him with the sedative. After she turned the lights back on, she led Drew to a stateroom to have sex with him. Her plan was thwarted when Julian walked in Kim and Drew. Julian realized what Kim had intended, so he quickly whisked her away then persuaded her to seek treatment.
The following day, Kim confessed to Drew that she had drugged him and why. Drew was disappointed, but he took comfort in knowing that Kim had decided to start therapy. As the weeks passed, Kim realized that she couldn't remain in Port Charles because there were too many painful reminders of her son. She persuaded Julian to pack up, sell his bar, and move to Manhattan with her. However, fate had other plans.
Ex-cult leader, Shiloh, had kidnapped Oscar's best friend, Cameron Webber with the intention of transferring Drew's memories prior to 2012 from a flash drive to the teenager. Franco Baldwin had stopped the mad science experience from being carried out on his stepson by taking Cam's place. Shortly afterwards, Franco woke up believing that he was Drew Cain. Franco's wife and the real Drew Cain tried to explain what had happened, but Franco insisted that he was the real Drew Cain even when a look in the mirror proved otherwise.
Kim was stunned when she ran into Franco at the hospital and he suddenly kissed her the way that Drew once had. Franco insisted that he loved her, and he shared poignant memories of her past with Drew that only Drew would have known. However, she was determined to start a new life with Julian. Franco refused to give up, even when Kim explained that Liz was her close friend. Franco didn't care because he believed that he was Drew -- the man who had loved Kim. Eventually, he wore her defenses down, and Kim and Franco made love. Kim decided to break things off with Julian and leave town with Franco, but Liz had gotten wind of their plans.
Moments after Kim and Franco rendezvoused on the piers, Liz showed up with two orderlies and a court order declaring Franco mentally incompetent. Kim watched helplessly as Franco was restrained, sedated, and then taken to General Hospital. The following day, Julian stopped by the apartment to help Kim finish packing. Kim stared at the unopened letter that she had left on the coffee table, but she said nothing as Julian talked about their future. Eventually, Julian noticed the letter, but Kim rushed to snatch it out of his hands as she tearfully explained that she believed her future was with Julian.
Julian quickly surmised that something had happened, and he questioned Kim about Franco. Reluctantly, Kim admitted that she had gotten caught up in the moment, and she had slept with Franco. Disgusted and hurt, Julian walked out on Kim without reading the note. After Kim pulled herself together, she went to work trying to find a lawyer to help Franco. He had been transferred to Shadybrook for treatment.
